I am sure a lot of guys died with that but were "cleaned up" by their families due to embarrassment which they shouldn't be. People need to know the risks involved in auto-erotic aphysixation and virtually all deaths were accidential - they were NOT suicidial, they liked the sensation when being "strangled" at the height of sexual climate. His former wife begged him to see a counselor about his sexual fetish that could be lethal. It's highly common in men, extremely rare in women. The reason is that for men who likes to be strangled at the height of climax, their erection is often more firm (blood vessels literally vasodilate trying to get blood).
I find no justification to judge him for that accident. He did it in private and he made his choice. It was just unfortunate he ended up like that. Remember, most men who practiced that act never died from it.
